Please Review the Diver Medical Questionnaire
Before participating in any diving activities, all guests are required to complete a Diver Medical Questionnaire on arrival.
To make your check-in smooth and avoid delays to your diving schedule, we recommend reviewing the form before your trip.
Diving places unique demands on the body, and this medical screening helps us ensure you are fit to dive safely.
If you answer “YES” to any of the medical questions, you will need written clearance from a doctor before diving.
We strongly encourage you to check this in advance, especially if you have any medical conditions, take regular medication, or have had recent surgery.
